Understanding change forms

What is a change form?

A change form is an updated version of one or more startwork documents that your production company needs you to review and re-sign. Change forms are issued when your production makes updates to your deal terms; such as your pay rate, allowances, or deal dates, after you've already signed your original startwork.

When will I receive a change form?

You'll receive a change form if your production company makes changes to your deal that affect your startwork documents. Common examples include:

  • A rate adjustment or new rate added to your deal

  • A change to your job title or union/guild affiliation

  • Updated deal notes, start date, or end date

  • Changes to your allowances

Not all deal changes will result in a change form; only changes that affect details tied to your signed startwork documents.

How will I know a change form is waiting for me?

When a change form is issued, you'll receive an email notification from Wrapbook. The email will include:

  • The name of the production company and project

  • The name of the production administrator who made the change

  • A direct link to your updated startwork documents

You can also find the change form by logging into your Wrapbook account and navigating to the project.

Where do I find and sign the change form in Wrapbook?

  1. Click the link in your notification email, or log in

  2. From your dashboard, click on the project tile for the relevant project

  3. You'll be directed to your startwork, where you'll see the updated document(s) waiting for your signature

  4. Review the document to confirm your updated deal terms

  5. Sign the document digitally

Can I submit timecards while my change form is pending?

No, your timecard access will be on hold until you've completed re-signing your change form. Once your signature is complete (and any required production counter-signature is completed), your onboarding status will be restored and you'll be able to submit timecards again.
