## Submitting a workers’ compensation claim in Wrapbook

Wrapbook provides workers' compensation coverage for employees and loan-out workers on projects with approved applications.

#### A worker just got injured on set — what should we do?

### Does the worker need approval before seeing a doctor for a workplace injury?

If they’re seriously injured, medical help should be sought immediately. Paperwork can follow.

- *Supervisor Statement*

### **I**’m filing a claim in California—what additional forms do I need?

California requires:

- DWC-1 Claim Form
- DWC-7 Notice to Employees
- MPN (Medical Provider Network) Notification

### How soon do workplace injuries need to be reported?

Immediately. Claims should be reported **as soon as possible** to ensure timely processing.

### Is there a deadline for sending Wrapbook the forms?

Yes. Please email all forms to [workcompclaims@wrapbook.com](mailto:workcompclaims@wrapbook.com) within **24 hours** of the incident.
